- Need Bachelors or Masters of Social Work and need to be licensed in Maryland
- Fully onsite
- 60-65k
Duties:
- Evaluate patients (existing, new and prospective) based on their needs, limitations and desires
- Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager
- Build rapport with patients and their families
- Coordinate care with PT and OT departments
- Develop care plans for patients
- Participated in annual, quarterly and significant change resident care conferences.
- Create and implement resident care plans based on the current bio-psychosocial assessment.
- Coordinate and record resident council meetings.
- Coordinate with resident, family and staff to review, revise and implement discharge plan
Requirements
- Must have bachelors Degree
- Must be licensed in MD
